Experienced Nurse – Unenrolled Service An exciting opportunity to be an integral part of the team delivering health services to the unenrolled population in the Hutt Valleywork close to Home - Hutt Valley based0.4 FTE, Fixed term role until 30 June 2025The nurse is an integral part of the small team we are establishing to deliver care to the unenrolled population in the Hutt Valley – including those living in emergency housing and homeless - and support the enrolment of complex clients to a general practice.
This work builds on experience gained through the delivery of Te Awakairangi Health Networks' Karanga Manaaki programme and work completed by Kokiri Marae, Te Ati Awa and Pacific Health Service to support unenrolled clients in their communities.

Mo te tunga | Role Overview We have an exciting opportunity for an experienced nurse to join our unenrolled service team.
As one of two nurses you support the delivery of our nurse-led and GP led clinics, providing clinical support, which includes:  Undertaking comprehensive nursing assessments of unenrolled patientsSupporting the clinical lead nurse to complete nursing tasks Providing education and support for increasing health literacy for patientsComplete referrals to a wide range of health and social servicesBeing able to communicate and engage with a wide range of different peopleBeing familiar with Microsoft Office software packages (Outlook, Word, Excel) and working knowledge of Indici Patient Management System is desirable Culturally responsive care aligned with kaupapa Maori principles.Having experience working in a community setting.Being able to prioritise tasks and manage the workday effectively Mou | About you Clinical experience working with people with long term conditionsAbility to engage and communicate in an appropriate and respectful manner with Maori, Pakeha, Pasifika and other cultures.Experience working with/engaging with vulnerable or hard to reach peopleAn understanding of primary and secondary health careExcellent written communication skillsAbility to prioritise tasks and juggle conflicting prioritiesA sense of humour and an ability to work as part of a teamWillingness and commitment to the principles of the Treaty of Waitangi and promoting positive outcomes for Tangata Whenua.To view the job description please visit pehea te tuku tono | How to apply Please note: we may review applications before the closing date and we encourage you to apply as soon as you are able.
