Experienced Nurse – Unenrolled ServiceAn exciting opportunity tobe an integral part of the team delivering health services to the unenrolled population in the Hutt Valleywork close to Home - Hutt Valley based0.4 FTE, Fixed term role until 30 June 2025The nurse is an integral part of the small team we are establishing to deliver care to the unenrolled population in the Hutt Valley – including those living in emergency housing and homeless - and support the enrolment of complex clients to a general practice.This work builds on experience gained through the delivery of Te Awakairangi Health Networks' Karanga Manaaki programme and work completed by Kokiri Marae, Te Ati Awa and Pacific Health Service to support unenrolled clients in their communities.Ko wai matou | About UsTe Awakairangi Health Network is a primary care network focused on achieving equity and improving the quality of our health services for whanau living in the Hutt Valley. We will do this by ensuring services are provided, developed and implemented in accordance with the Articles of Te Tiriti and the principles - Tino Rangatiratanga, Equity, Active Protection, Partnership and Options.Mo te tunga | Role OverviewWe have an exciting opportunity for an experienced nurse to join our unenrolled service team.As one of two nurses, you will support the delivery of our nurse-led and GP-led clinics, providing clinical support, which includes:Undertaking comprehensive nursing assessments of unenrolled patientsSupporting the clinical lead nurse to complete nursing tasksProviding education and support for increasing health literacy for patientsCompleting referrals to a wide range of health and social servicesCommunicating and engaging with a wide range of different peopleBeing familiar with Microsoft Office software packages (Outlook, Word, Excel) and having working knowledge of Indici Patient Management System is desirableProviding culturally responsive care aligned with kaupapa Maori principlesHaving experience working in a community settingPrioritising tasks and managing the workday effectivelyMou | About youClinical experience working with people with long-term conditionsAbility to engage and communicate in an appropriate and respectful manner with Maori, Pakeha, Pasifika and other culturesExperience working with/engaging with vulnerable or hard-to-reach peopleAn understanding of primary and secondary health careExcellent written communication skillsAbility to prioritise tasks and juggle conflicting prioritiesA sense of humour and an ability to work as part of a teamWillingness and commitment to the principles of the Treaty of Waitangi and promoting positive outcomes for Tangata WhenuaMe pehea te tuku tono | How to applyPlease note: we may review applications before the closing date and we encourage you to apply as soon as you are able.
