We are for Clinical Team Manager to oversee our Hertaunga clinic.You need to be a motivated and experienced Registered Nurse and/or Nurse Practitioner, who has:· strong people and leadership skills and · a genuine desire to provide safe, effective healthcare, health promotion and preventative health strategies.The community-based clinics are designed to support equity of access, experience, and outcomes for whanau needing primary medical care. They are intended to be whanau-led and centred services that will be supported by a mix of clinical (Nurses & Nurse Practitioner ) and non-clinical kaimahi (Health Educators, Health Coaches and Administrator). The service will also be delivered from both fixed and mobile sites whether for community events or providing care in remote/rural communities.The model of care key foundations include:whanau-centred and led;integrated care; inter-professional practice; innovation;digitally enabled deliveryThe clinics are designed to be outcomes and results focused.
