Would you like to be part of a Healthy Ageing Service - Te Ara Tiaki Pakeke, committed to supporting older people to Get Well, Stay Well and Live Well in supportive communities? Are you a registered health professional with at least three years' experience with a current annual practising certificate? We are looking for a person to join our service to undertake Needs Assessment and Service Coordination (NASC) . The key objective of NASC is to maximise and support the ability of older people (usually over 65 years, though can be younger) to identify their needs and goals and make decisions on how these both can be met. NASC assess needs, identify support options and access support services in support of a client. NASC work with clients and their whanau (family), together with other stakeholders to build integrated supportive plans that focus on maximising a person's independence so that they can participate as fully as possible in society, in accordance with their goals, culture, resources, and abilities. This role may be fulfilled by Registered Nurses, Occupational Therapists, Physiotherapists and Social workers. The remuneration range for this role is $59,035 - $103,630and is covered by the collective agreement: PSA Allied MECA Allied, Public Health & Technical& NZNO MECA (2 Aug 2020 – 31 Oct 2022)Position Description | Needs Assessor & Service CoordinatorPosition Description | Registered Nurse, NASCMou - About youTo be successful in this role you will have a commitment to listening to and advocating for the voice of services users, undertaking assessment to identify their needs, with strong relationship and networking skills to create integrated collaborative client centred plans. You will enjoy working in a busy work environment where you work independently, utilising strong planning and organising skills. Te Whatu Ora Tairawhiti has a commitment to TeTiriti of Waitangi, and to recognising Te Ao Maori perspectives and practices. A desire to address inequity for Maori, with strong cultural competencies would be advantageous.
