Te Mahi/About the Position The Needs Assessor role is focused on liaison and advocacy, partnering with patients and their whanau to create a support care plan when living circumstances and care needs are changing.
He momo/About You Important to this role is the ability to connect and assess, undertaking interRAI or support needs level assessments in either the patient's own home or in the Nurse Maude Hospice In-Patient Unit.
Developing and maintaining excellent working relationships with other community health professionals providing palliative care is essential.
If you have 4 years post-registration relevant experience as a registered health professional (nurse/allied health) with a current practising certificate, excellent communication skills, and a full driver's license, you may be who we are looking for.
