Ashburton & Rural Health Services provides a range of Inpatient, Day patient, Outpatient and Community based health services to the community.The Community Service team at Ashburton Hospital has an opportunity for a Registered Nurse/Allied Health Professional to join their team as a Needs Assessor and Service Coordinator for Older Persons' health.This role requires a comprehensive clinical assessment of older persons' medical, rehabilitation and support needs to enable them to remain in their current environment.
You will be supporting older people to maintain their independence using a restorative model at home by providing necessary support or assisting them in the decision-making process around placement.
You will liaise with the multi-disciplinary team, coordinating referrals and empowering the client and their family to ensure a positive outcome.This role will require you to use the electronic InterRAI Assessment tool.
InterRAI trained is preferred but not essential, as training can be provided.We have a 0.6FTE fixed term position available until the end of October 2025, specific days to be discussed.Applications are only accepted online, so please visit our website at to complete an application.
