NAVIGATE CARE MANAGER
Navigate Is a Regional Intellectual Disability Supported Accommodation Service (RIDSAS). We work with people with an intellectual disability who have complex behavioural needs and/or who have offended.
About the role | He korero whakamarama mo tenei turanga
The Care Manager is legally entrusted with the compulsory care and rehabilitation of individual care recipients. They are responsible for developing and implementing an individual's care and rehabilitation plan and for ensuring regular clinical reviews are undertaken.
Our RIDSAS services focus on the provision of intensive rehabilitative support to enable people we support to develop the skills they require to participate in and re-join their communities. This role will work in partnership with a multi-disciplinary team of disability/health practitioners and other related professionals. This is a full-time position, Monday through Friday, with occasional on-call phone duty scheduled every 10 weeks.

What you will bring | Nga tohungatanga mo tenei turanga
Tertiary qualification in a related Health field e.g., nursing, social work, occupational therapy, psychology.
Sound understanding of the responsibilities and requirements of the Intellectual Disability (Compulsory Care and Rehabilitation) Act 2003 (IDCCR Act).
Knowledge and understanding of the intellectual disability sector, Health & Safety legislation and the Health and Disability Sector Standards.
Extensive experience supporting clients with complex behavior issues.
Proven expertise in client centered planning.
Excellent communication skills, written and verbal.
Ability to develop working relationships with a wide range of family, community, professional and employee groups.
High levels of resilience, personal integrity, and judgement, with the ability to problem solve and respond effectively under pressure.
Extensive interpersonal skills, with the ability to motivate staff/teams.
An understanding and commitment to Te Tiriti o Waitangi.
Hold a current driver's license and be able to drive an automatic vehicle.
