Rotating shifts from Monday to Sunday
Shift hours: 7:00 am - 3:30 pm and 3:00 pm - 11:30 pm

Responsibilities
Apply clinical expertise to ensure the safe and effective delivery of services to clients
Evaluate and manage potential and actual risks to clients, ensuring thorough documentation, reporting, and communication
Manage client records from entry to exit, including conducting Client Pathway assessments and Support Needs Assessments
Provide clients with the information they need to make informed decisions
Skills & Experience
Current and Full registration with a Professional Body as per HCPA 2003 Act, Social Worker Registration Act 2003, or DAPAANZ (essential)
At least 2 years of post-qualification experience in the Mental Health/AO sectors (preferred)
Proven experience and knowledge in developing personalised care plans
A valid full driver's licence and permanent working rights in New Zealand
