The role of the Primary Care Liaison Nurse is to work across primary and secondary health settings to ensure clients are able to access the treatment most appropriate to their needs. The GP Liaison Nurse does not provide a care management role to clients, but rather acts in a consultation role for primary care via GPs, Health Centres, and Community Mental Health teams, as well as providing assessment and brief intervention when required. Mo to matou ropu - About our team The Primary Care Liaison Service is a small team of experienced clinicians covering the MHAIDS region. We work in both the primary and secondary health settings to support tangata whaiora receiving the right care, at the right time, in the right environment. Mou - About you You will be a registered nurse with extensive experience in mental health settings (at least 3 years), including in the community. Able to work autonomously as well as contribute to the MDT. Creative, innovative thinking, and problem-solving. Application of best practice models of care. A willingness to learn and develop your practice. Excellent communication and interpersonal skills. Self-motivated and skilled in developing relationships with a variety of other professionals. You will hold a full NZ driver's licence.
