About the Role – Community Mental Health Registered Nurse, He Waahi Kainga
We have a permanent position available within He Waahi Kainga Service.
He Waahi Kainga is a service that provides support to whanaunga transitioning from Henry Rongomai Bennett Centre into the community. This includes wrap-around support for whanaunga including creating whanau plans, advocacy and connecting whanau to life skills services that support future housing aspirations and their ongoing wellbeing and needs. You will need to have a passion for working alongside our Mental Health whanaunga so having a greater understanding of the needs of our whanaunga is important. Having great administrative skills and attention to detail is also important as this role does require a lot of admin work.
The Community Mental Health Registered Nurse provides comprehensive mental health care within the community setting. This role involves assessing, planning, implementing, and evaluating mental health care for individuals and families. The RN collaborates with a multidisciplinary team to deliver effective and culturally appropriate mental health service.
Position details
Monday to Friday, 8.30am – 5.00pm. Please note on occasions you may be required to work outside of these hours, depending on the support needs of whanaungaStarting from $80,000 - $90,000 per annum (depending of qualifications and experience) A day in the role
Conduct comprehensive mental health assessments and develop individualised care plans using standardised tools and clinical judgement.Provide evidence-based nursing interventions, including medication management, counselling, and therapeutic support, while monitoring patient progress and adjusting care plans as needed.Collaborate with psychiatrists, psychologists, social workers, and other healthcare professionals to ensure integrated care and coordinated referrals to necessary services.Educate patients and their families about mental health conditions, treatment options, and coping strategies, while guiding them through available community resources.Deliver crisis intervention and stabilisation, developing and implementing safety plans for individuals experiencing acute mental health crises.Maintain accurate, confidential documentation of patient assessments, care plans, and progress notes, while ensuring compliance with clinical governance requirements.Provide culturally sensitive care tailored to the needs of the community, engaging with Maori and Pasifika communities to offer appropriate services.
