Whakarongorau aims to be there when face to face health and social services cannot, providing consistent, clinically supported access to services for those in need or when people are unable to access other options due to location, time of day, financial or cultural barriers.Job DescriptionWhakarongorau supports a range of primary care clinics across Aotearoa, providing continuity of care for primarily high-needs rural communities and priority populations.This is a contract role for clinics (Monday – Friday primarily).Provide care tailored to meet the unique needs of registered patients, collaborate with clinics to deliver optimal healthcare services, conduct remote telehealth consultations and understand local services and work to overcome access barriers.QualificationsYou will be a Matanga Tapuhi (Nurse Practitioner) with appropriate experience in Primary Care who is passionate about delivering high-quality care to rural, high-needs and our vulnerable communities, who face many challenges and barriers to access quality health care. You will be committed to providing compassionate, culturally responsive, patient-whanau centred care, and able to work collaboratively with the clinic team.Strong communication, relationship-building & teamwork skillsExperience in remote work is preferable but not essentialCommitment to a recurrent rosterIntegrate understanding and learnings of Te Ao Maori, Tikanga and Te Reo into approach and delivery of careTo succeed in this role, you will need:A high-speed fibre internet connection & a high-spec computer capable of handling high-demand softwareA multi-screen setup, headset, printer, and scannerYou will need a private space to work from that ensures the confidentiality of conversations, documents, and technology.Additional InformationE kimi ana matou i etahi tangata e kaha whakanui i enei uara - We are looking for people who align naturally with these values and will champion themMahia te mea tika / Do the right thingHihiri e te kounga / Motivated by qualityPokohiwi ki Pokohiwi / Shoulder to shoulder
