Integrated Services Case Manager - Lower HuttWe have an exciting opportunity for two Integrated Services Case Managers to join our team, based in Lower Hutt. You will also be supported to bring your best self to your mahi.You will need to be open-minded and willing to adapt your thinking at a moment's notice. This is a fast-paced role so thinking ahead, being organised and prioritising your work will be important traits to bring.The clients you will work with have very complex needs and can display behaviours that are challenging, so you will need to have strong resilience, communication skills, and be able to de-escalate complex situations.Being able to provide strong case management to these clients is important as you work towards helping them with their complex situations/behaviours with the goal of returning them to face-to-face case management or employment.About the RoleFor this role, you will work in a small team providing day-to-day case management to a group of clients deemed high-risk. Interaction with this group will primarily be by phone, email, and letter correspondence. This approach looks at a client's circumstances holistically and works with them to provide wrap-around support.The successful applicant will strive to provide a service that is empathetic, open, and courteous, ensuring understanding and trust. Manaakitanga is the norm in your practice.You will also need to provide ongoing monitoring and reporting on progress and outcomes achieved for each client. You'll need to demonstrate proficiency in written communications, always maintaining a professional and high standard.You will need to have utmost confidentiality and showcase your skills as an experienced Case Manager.Technically, your knowledge of Work and Income products and services, relevant legislation, and policies is advanced. You strive to provide a service that is empathetic, open, and courageous, ensuring our MSD values are top of mind in your practice.We are committed to providing a safe and supported environment for you to work in, along with strong security provisions that will be put in place on your behalf. Managerial support, professional supervision, and training will also be provided.To be successful in this role, you will have:Exceptional personal resilience.Exceptional skills to engage with people with complex needs and behaviours.Sound judgement and decision-making abilities.Strong networking and relationship skills – you will need to have a calm demeanour and be able to apply common sense.Good computer skills and the ability to pick up new systems and processes.Strong written communication skills and familiarity or direct experience with responding to complaints and escalated issues.If you have exceptional resilience, excellent communication skills, and a focus on continuous improvement, we'd like to hear from you.Remuneration starts from $78,222 (Band 5, Step 1).
