About the Role
As a Home Care Services (HCS) Co-ordinator, you will provide clinical oversight and coordinate care services for clients. You will work closely with clients, their whanau, and support workers to ensure culturally appropriate, professional care that enhances client independence and participation in community life.
This role is a permanent, full-time role, offering regular business hours (0800am - 1630pm).
Monday to Friday, with no overnights or weekend work.
Mobile role with lots of home visits.
Whakatane and Tauranga roles available.
Four weeks of holiday leave and 10 days sick leave.

Key Responsibilities
Provide clinical oversight and coordinate culturally appropriate care plans.
Maintain accurate and up-to-date client records, reflecting individual needs and backgrounds.
Identify and support training needs for support workers, with a focus on culturally responsive care.
Ensure services are delivered in a timely, professional, and culturally sensitive manner.
Complete regular reporting and ensure contractual compliance.
Participate in staff training and adhere to organisational policies, respecting diversity and inclusivity.
Skills and Experience
Must be a Registered Nurse, Occupational Therapist, or Physiotherapist, with current registration from the NZ Nursing Council, NZ Physio Board, or NZ Occupational Therapy Board.
Relevant experience in clinical or home care coordination is necessary.
Practical experience in community settings, with a strong understanding of the needs of elderly and disabled clients.
Strong communication and interpersonal skills, with the ability to work independently and within a team.
A commitment to culturally sensitive care and an understanding of the diverse needs of clients.
