Health Navigator

Details of the offer

As a Health Navigator at Te Mana Oranga Trust, you will play a pivotal role in improving health outcomes for the local community. Although this full-time position is based in Kaikohe, Northland, you will be responsible for supporting and guiding individuals and families to access the healthcare services they need. Much of this work could include picking people up from their homes in the Kaeo area and transporting them to Kaitaia, Kerikeri, Kawakawa or Whangarei for appointments.
What you'll be doing
Assisting clients to navigate the healthcare system and access appropriate services, including general practice, medical specialists, community health providers and social services
Providing one-on-one support and advocacy to help clients overcome barriers to accessing care
Connecting clients with relevant resources and programs
Collaborating with healthcare providers and community organisations to coordinate holistic care
Maintaining detailed client records and preparing reports
Participating in team meetings and professional development activities
Lots of driving, picking people up for appointments so you will be one who loves to chat
What we're looking for
Level 4 Mental Health and Wellbeing Certificate is the minimum qualification
Demonstrated ability to build rapport and work effectively with people from diverse backgrounds
Strong communication and interpersonal skills, with the capacity to liaise with healthcare professionals
Excellent organisational skills and attention to detail
Commitment to improving health equity and advocating for vulnerable populations
Cultural competence and understanding of local community needs
Experience working with sufferers of mental health and addictions
New Zealand residents only - we will not be sponsoring anyone from overseas
The successful applicant will be required to undergo a Police check
