About the Role
The organisation is seeking a Health Improvement Practitioner to join their general practice team. The role involves delivering brief, consultation-based services to practice team members and patients, with a focus on evidence-based behavioural interventions for both mental and physical health conditions. The practitioner will support an integrated 'Model of Care,' facilitating smooth transitions between general practice, community-based NGOs, and specialised services. Provide brief, evidence-based behavioural interventions in individual, group, and whanau settings
Collaborate with the GP team to deliver consultation-based services
Guide clients through available services to support their overall well-being

About You
To be considered, you MUST have:
Registration with a governing body such as the Social Worker Registration Board, Occupational Therapy Board, or New Zealand Association of Counsellors
A current practicing certificate as a registered health professional
Clinical experience in mental health, with skills in delivering psychological interventions
Understanding of behavioural medicine and the connection between medical and psychological systems
Strong relationship management abilities and familiarity with the healthcare system
Excellent communication skills and the ability to work with diverse populations
Strong organisational skills and a proactive approach to planning
