We are looking for highly motivated individuals who are enthusiastic about helping people manage and improve their health. Health Coach roles may suit someone who has been a peer support worker, volunteer, or has lived experience supporting a whanau member with a long-term condition such as diabetes. Working as a Health Coach offers a busy but rewarding environment, where you will tangibly see the difference and positive impacts that you can have on people's lives.There are five main areas of focus for the Health Coach:To provide support for their client to manage their condition themselves;To bridge the gap between their client and their GP;To help their client find their way around the healthcare system;To offer their client emotional support;To be their client's ongoing first person to contact if they have a question around their health care.We are after an individual with the following skills and attributes:Basic understanding and knowledge of health conditions (hypertension and diabetes in particular).Having lived experience in the health sector is a bonus.Good caseload management - you will be responsible for managing your caseload while working collaboratively with the wider team.Prior training or certificates relevant to the role are required.Good IT skills as you will be required to enter clinical notes into the Patient Management System in a timely manner.Genuine empathy and willingness to support people to work on their health.Good understanding of behavioural change techniques.Experience in skills of problem solving, self-management, goal setting, and creating action plans.Confidence in reaching out to patients.Culturally aware, and aware of the New Zealand health context.About Us: The Cause Collective is a community organisation that aims to create the conditions for communities to thrive from this generation to the next.
