Health Hawke's Bay – Te Oranga o Te Matau-a-Maui is the region's single Primary Health Organisation (PHO), covering a population of approximately 170,000. Ko wai Poutoko? |What is a Health Coach?
Health Coaches work within a designated general practice or practices as an integral part of the team providing behavioural health interventions and self-management support to assist individuals and whanau to achieve their health and wellbeing goals.
As a Health Coach you provide half hourly sessions with people experiencing any thoughts, feelings or behaviours that are impacting their overall wellbeing. The support you provide will be holistic in nature and can be related to peoples physical, mental, social and spiritual wellbeing journey. This can include assisting lifestyle changes such as smoking cessation & exercise and promoting self-management of long-term conditions like diabetes.
The Health Coach role is busy yet rewarding, where you will tangibly see the positive impact you are having on peoples lives.
Ideally you will:
Have a Level 4 Health and Wellbeing Certificate (completed or working towards) OR significant relevant work experience
Relevant experience within the health and wellbeing sector (e.g. community health, health promotion or long-term condition management)
Have experience engaging effectively with Maori and Pacific peoples
Have knowledge of the Te Tiriti o Waitangi, be an advocate of our Ka Hikitia strategy & willing to embrace tikanga Maori
Be open to change and motivated to implement new ways of working
Have good digital literacy, organisational and time management skills
Learn quickly and be able to adapt to ever changing situations
Be experienced in practicing autonomously
Be an excellent communicator and work in a collaborative, respectful, supportive and sensitive way
You will have a genuine interest in improving health outcomes for Maori and other underserved populations in our community, as well as a desire to understand and implement Maori models of care.
If you are the successful candidate, you will be required to undertake our mandatory training via Te Pou to be fully endorsed as a Health Coach.
