Supporting change and improving Primary Health service deliveryThis pivotal role leads the service support for Mahitahi member General Practices in our primary healthcare sector across Te Tai Tokerau. As a role that carries significant variation, responsibility, and scope for change, it will call on your extensive RN experience and clinical skills and knowledge to support a sustainable primary care future.Mahitahi Hauora is a primary health entity underpinned by Te Tiriti o Waitangi. Our Kaupapa includes ensuring whanau and communities are able to achieve self-determined wellbeing, access to the services they determine they need, and to live a long and healthy life. As a confident and experienced RN, you will lead the Neighbourhood Healthcare Homes programme across Tai Tokerau, support the implementation of new initiatives and programmes through general practice, and provide leadership and guidance in areas including education to build capability, Foundation and Cornerstone Accreditation, and change methodology and quality improvement that enhances service delivery to our enrolled population.Ideally, you will be familiar with primary healthcare and general practice challenges and enthusiastic about supporting general practice in providing sustainable models of healthcare and access to services, with a focus on equity.The role requires someone with:A current Nursing Registration and significant post-graduate studyMinimum of 5-10 years' experience in secondary and primary healthcareStrong knowledge of primary care sector models of care and programmesThe ability to develop strong relationships with health professionals, clinicians, and partners in primary careExperience in leading people through change management and quality improvementHigh level of IT competency including MS suiteExcellence in verbal and written communicationsExperience in leading and motivating teams to successful outcomesNote:You must have an unencumbered NZ Driver Licence and be a NZ Citizen or Resident to be considered for this role.Health professionals will be supported to maintain their Annual Practising Certificate.This is a full-time permanent position.If you have the above skills and experience along with the passion and commitment to supporting and developing primary healthcare, then please apply by:Downloading and completing an application for employment form #J-18808-Ljbffr
