ProCare has launched the Comprehensive Primary Care Team (CPCT)- a pioneering initiative designed to improve health equity and expand access to essential services in our communities.We're seeking a General Practice Pharmacist for a fixed-term role through to June 2025 in our integrated health clinic in Papakura. This is a 0.6 FTE (24 hours per week) position, and we're looking for someone to hit the ground running and make a real difference from day one!Why Start NOW?By joining the CPCT, you'll be part of an innovative project that's transforming healthcare delivery across Auckland, Aotearoa. This fully-funded model integrates allied health, social work, and pharmacy professionals into general practices to ease pressure on GPs, provide holistic care, and improve access for at-risk communities. The funding is secured through June 2025, giving you the chance to be part of something groundbreaking, starting right away!Your Role:As our Practice Pharmacist, you'll play a crucial role in:Managing medications and providing expert advice to ensure safe, effective treatment.Leading educational sessions on pain management and safe medication use for patients.Collaborating with a dynamic interdisciplinary team to improve patient outcomes and drive quality improvements in care.What We Need:A current APC with no restrictions.Post-graduate studies in clinical pharmacy or pharmacist prescribing (or enrollment).At least 2 years of experience as a registered pharmacist (primary or secondary care preferred).A passion for improving healthcare equity and experience working with diverse communities.Strong understanding of Te Tiriti o Waitangi and its implications in primary care.Why ProCare?ProCare is a leading healthcare provider that aims to deliver the most progressive, pro-active and equitable health and wellbeing services in Aotearoa.
