ProCare have launched Comprehensive Primary Care Team (CPCT), a pioneering model aimed at enhancing health equity by expanding service access through innovative roles within existing general practice teams. This initiative is full funded through to end of June 2025 and integrates allied health, social work, and pharmacy professionals into practices to better serve at-risk communities. Funding supports free patient access to essential services, easing pressure on GP teams while improving on-site family-focused care As part of this initiative, we invite you to join our small family-centred clinic, in Manukau City as a Practice Pharmacist . This role is 0.4 FTE (16 hours per week) and fixed-term through June 2025 . As our Practice Pharmacist, you'll be the go-to expert on all matters related to medicines. You'll connect directly with patients, offering personalized advice and conduct engaging educational sessions on pain management and safe medication use. As a CPCT Practice-based Pharmacist, your responsibilities will include: Managing and optimizing medication usage and reconciliation Conducting clinical evaluations to ensure best practice effectiveness Leading medication audits and quality improvement initiatives for equitable access Developing and managing standards and policies for safe and effective medication use Collaborating within an Interdisciplinary Team (IDT) framework to enhance patient care Fostering partnerships with other health and wellbeing providers We welcome Pharmacists with varying levels of experience to apply, but successful candidates will typically possess: A current APC with no restrictions Enrollment in or completion of post-graduate studies in clinical pharmacy or pharmacist prescribing At least 2 years of experience as a registered pharmacist in primary or secondary care settings An understanding of access inequities and health outcomes in Aotearoa, New Zealand, with knowledge of Te Tiriti o Waitangi and its implications for primary care Strong cultural competency and familiarity with the diverse communities we serve Experience in managing long-term conditions through medication management is highly valued.
