Details of the offer

ProCare is excited and proud to be launching an innovative new primary care workforce model, the Comprehensive Primary Care Team (CPCT). These roles are aimed at driving more equitable health outcomes by increasing overall access to services, through integrating a range of new role types into existing general practice teams.
This new, fully funded model integrating CPCT into existing general practice teams is intended to help reach more of our at-risk communities, reducing barriers to accessing care by bringing different allied health, social work, and physiotherapist-based professionals directly into the practices. The new funding for this pilot will allow for free access for patients to a range of crucial services, alleviating some of the strain on the wider GP team. This is intended to enable better integration of on-site whanau-facing care, while also offering much needed additional capacity and workforce to support our heavily stretched primary care teams.
The funding associated with this exciting new model also has dedicated workforce development investment and as such, can help fund chosen specialist pathways for each of the profession types.
We are now seeking Expressions of Interest from any relevant professionals eager to find out more about working at the frontline of community care.

About the Role The primary purpose of the role is to provide front-line assessment and care interventions as an integrated member of the general practice inter-disciplinary team. Working closely with the GP's, nurses and the wider team, the CPCT Practice Paramedic duties will include:
Telephone triage Telehealth and face to face consultations Issue specified medications under standing orders Order lab tests (under GP name) and x-rays Suturing and wound management Prioritisation of people and whanau to support hospital avoidance, early supported discharge from hospital and those at greatest risk of poor health outcomes.
We are interested in hearing from Paramedics with a range of experience levels and background at this stage, however our successful applicants will typically need to have:
Bachelor of Health Science in Paramedicine Current registration and Annual Practicing Certificate (APC) from Te Kaunihera Manapou Paramedic Council Ideally completed a post graduate diploma in health science paramedicine or having completed a paper in Community and Remote Area Paramedicine OR Be working toward a Post graduate diploma in health science paramedicine (or equivalent CAA accredited program). Comprehensive understanding of inequities in access and health outcomes in Aotearoa, New Zealand and detailed knowledge of Te Tiriti o Waitangi and its application to shaping all efforts within primary care Strong cultural competency and confidence with sufficient knowledge of the peoples, languages and cultures that form our most at-risk communities Excellent critical appraisal skills and being able to identify the best evidence-informed solutions to clinical and practice questions and issues.
