Are you passionate about making a positive impact in the lives of our elderly and ensuring they have access to appropriate local health and social services? Established in 1995, our client Tuwharetoa ki Kawerau Hauora are looking for a trustworthy, confidential, well-organised person to join their team. Our client provides quality health, social and education services for all who reside within Kawerau and the Eastern Bay of Plenty region.

He aha te tunga | About the role
As Elder Response Service Practitioner, you will manage approximately 10-15 cases per quarter, ensuring seamless delivery of support services to the Uri Haumate (uri) of Tuwharetoa ki Kawerau Hauora. Your primary goal is to ensure that older individuals experiencing, or at risk of, abuse and neglect have timely access to appropriate local services to ensure their safety and empower them to have greater control over their lives.
Key responsibilities of the role:

Implement interventions and strategies that reduce the risk of social isolation, neglect and/or abuse
Build rapport by working collaboratively with uri and their whanau, including caregivers, to develop goals and strategies to keep uri well
Identify and monitor any health and wellbeing issues
Prepare referrals to ensure kaimahi respond to immediate crisis promptly
Ensure clients are fully informed and involved in decisions
Work collaboratively with whanau and caregivers to empower and support uri
Identify needs and prepare referrals to external agencies for additional services
Provide education and health promotion to strengthen uri well-being and resilience
Complete reporting requirements, ensuring strict client confidentiality
File management, capture date and update progress details
Ensure all documentation is accurate and are managed and filed as per protocol

Minimum of three years' experience in a similar role
Proven experience working in a kaupapa Maori environment
Knowledge of TKKH and the Eastern Bay of Plenty Communities
Strong relationship and network management
Excellent organisational skills, with the ability to meet deadlines
Excellent computer literacy skills, with proficiency in all Microsoft Office functions
Be positive, adaptable to change and resilient
Knowledge of Tuwharetoa ki Kawerau tikanga and kawa is desirable
Competency in Te Reo Maori and a good understanding of tikanga Maori is desirable
Registered practitioner/social worker with a current practicing certificate would be desirable but not essential
