District Nurse - Triage - Community Clinical Services at Te Whatu Ora - Health New Zealand Te Tai Tokerau, Northland Whangarei Permanent full and part time We have an exciting new opportunity for highly motivated Community Nurses to join our Community Clinical Services team primarily based in Whangarei. There may be an option to work from Bay of Islands Hospital some days.
This is a rewarding role with opportunities to make a difference in the community you belong to.
We welcome applications for both full time and part time positions.
This role requires flexibility, critical thinking, comprehensive assessment skills and ability to prioritise patient care. The primary function is to support the district nursing teams by providing full referral data and appropriate advice.
This will be achieved through:
Assessment of referrals receivedGathering and collating other information needed to support the patient's referral.Providing support to the referring teams (from inpatient and community settings) to facilitate appropriate referrals.Te Whatu Ora Te Tai Tokerau is committed to Te Tiriti o Waitangi and holds stewardship for equitable health outcomes for Maori as Tangata Whenua as the highest priority.
Key requirements: Registration with Nursing Council of New Zealand and a current Annual Practicing Certificate.Experience in community health care is desirable.Collaborates with fellow team members and work groups to achieve specific objectives.Communicates information effectively and accurately, both orally and in writing.Shortlisting may occur prior to the close date so please do not delay and apply now!
