Digital and Administration Support - Proactive Care TeamTaunton Deane West Primary Care Network (PCN) is seeking an experienced Digital Coordinator and Administrator to support the delivery of a developing Proactive Care Service.
This part-time role (24 hours per week, 12-month fixed-term contract) involves managing IT administration, overseeing service operations, and fostering positive relationships with stakeholders, surgeries, patients, and other services.
The successful candidate will ensure all tasks are completed in accordance with established procedures, protocols, and timelines.Main Duties of the JobDeliver and manage the developing Proactive Care Service, utilising digital transformation to improve patient care, outcomes, service experience, and cost-efficiency.Enable patients to manage their health and wellness through technology, including communication tools for managing illness, health risks, and medication.Develop and maintain relationships with stakeholders, including internal teams, neighboring practices, patients, and external services.Evaluate and promote the service both internally and externally to ensure continuous improvement and adoption.Manage TDW PCN IT Coordination, including extensive EMIS searches and training for new PCN and Practice employees.Work with digital tools e.g., AccuRx, online access, video consulting to support the transformation of primary care services.Collaborate with stakeholders to share best practices and drive service development across networks.About UsTaunton Deane West Primary Care Network is located in Somerset and is formed by three GP Practices: Luson Surgery, Wellington Medical Centre, and Lister House.
Within the PCN, the three practices work closely together to improve the quality of care that we provide in alignment with the needs of our patient population.
The PCN has a range of additional staff roles who work alongside Practice staff to help support all of our patients.
The team consists of Care Coordinators, Complex Care Nurses, First Contact Physiotherapists, Occupational Therapists, Pharmacists, Mental Health Link Practitioners, Digital Data Coordinators.Job ResponsibilitiesThe role will focus on the delivery of a developing Proactive Care Service.
The aim of the service is to bring digital transformation to health, supporting holistic care and clinical outcomes, service and patient user experience, in addition to cost saving, consistency, and fewer clinical errors.
The use of technology will give the patient control over their own healthcare, utilising information and communication through technology to manage illness, health risks, and medication.
The role requires someone to not only deliver the service but to proactively develop relationships with stakeholders, evaluate the service, and help promote the service internally and externally.The post holder will manage TDW PCN IT Coordination, including management of running extensive EMIS searches and training for new PCN and Practice employees.
They will work with data for digital tools to enable transformation of primary care services such as AccuRx, online access, and video consulting, but not limited to those listed.
The idea is to include plans for a population health management approach.The post holder will be responsible for developing and maintaining relationships with different stakeholders and linking in with neighbouring Primary Care Networks to share best practices.Please see attached Job Description for key responsibilities.Person SpecificationExperienceKnowledge of Primary CareExperience of working in a multi-disciplinary settingAble to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentialityExperience of administrative/coordinator dutiesKnowledge/familiarity with medical terminologyUnderstanding of current issues facing the NHSUnderstanding of health and social care processesExperience in use of databasesInterest in digital change, and experience of EMIS, data handling and analysisPR and Marketing skills to aid promotion of the serviceQualificationsNVQ Level 2 or equivalentWilling to work towards NVQ Level 3Skills and AttributesProven record of excellent written and verbal communication skills and interpersonal skillsExcellent organisational and admin skillsAnalytical skills and ability to interpret information and present it in a clear and concise mannerEvidence of excellent knowledge and competence with Microsoft OfficeAble to deal with service users sensitivelyAble to work effectively as part of a teamAble to prioritise and manage own workload with minimal supervisionWillingness to undergo further training and committed to own developmentCar user as travel between sites across the Taunton area will be requiredAbility to understand and use social mediaExperience providing advice/signposting to usersExcellent motivational and influencing skillsDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.HR Advisors to the Taunton Deane West PCN
