This new Health Coach role will primarily work with patients and the families of those who have been diagnosed with diabetes and prediabetes. The role will focuse on diabetes reversal/remission/good management and prevention with other family members. Talanoa around mental health, providing emotional support and linking families into other services relevant to wellness is also a key element of the role.About you:You will be a courageous, passionate and trained Health Coach ready to tackle the rising tide of type 2 diabetes in our Pacific families and communities. You will understand that families are capable of sustained change that result in good diabetes management and/or diabetes remission/reversal and the prevention of diabetes.
