Psychology, Counselling & Social Work (Healthcare & Medical)Full timeAdd expected salary to your profile for insightsCourageously tackling type 2 Diabetes in our families and communities.A Pacific model of care reflecting values, beliefs, and ways of working.Integrated role in General Practice based Manurewa.About the role:We are on a mission to tackle the rising tide of type 2 Diabetes in our Pacific families and communities.In collaboration with our dedicated team of GPs, Nurses, Care Co-ordinator, Mind Health Practitioners, Community Support Navigators, Psychiatrists, Psychologists, Cultural Navigators, and Whanau Ora, we seek a brave and passionate Health Coach to join us on our mission.The Health Coaches in our practice support patients and their families to achieve wellness goals through the development of skills, tools, confidence, and healthy lifestyle choices. This new Health Coach role will primarily work with patients and the families of those who have been diagnosed with diabetes and prediabetes. The role will focuse on diabetes reversal/remission/good management and prevention with other family members. Talanoa around mental health, providing emotional support and linking families into other services relevant to wellness is also a key element of the role.About you:You will be a courageous, passionate and trained Health Coach ready to tackle the rising tide of type 2 diabetes in our Pacific families and communities. You will understand that families are capable of sustained change that result in good diabetes management and/or diabetes remission/reversal and the prevention of diabetes.
