The Day Programme Coordinator is an important part of our professional and expert multidisciplinary team who collaborate and take pride in patient-centred, holistic care. Mo te tunga | About the role The Day Programme has been developed to support and empower patients to enhance their quality of life whilst living with a life-limiting condition. You will collaborate with the multi-disciplinary teams to identify patients' needs, then coordinate and facilitate the day programme, in order to meet those needs. You will be expected to grow, adapt and enhance the programme to match the constantly changing requirements of the patients and communities we care for.
He korero mou| About you An experienced health professional, you'll bring a good understanding of holistic care and how to use this in a palliative care setting to get the best outcomes for patients. You'll have experience facilitating or leading groups and enjoy creating a sense of community, affinity, and support.
You will also have:
Excellent planning, time management and organisational skills Cultural awareness and sensitivity in dealing with people from diverse backgrounds and ethnicities Ability to build relationships across multiple sites Strong facilitation and communication skills Sound IT skills Ko matou tenei | About us Harbour Hospice is one of the largest hospices in Aotearoa and has been caring for turoro (patients) and their family and whanau for more than 40 years. Every day we care for close to 400 people across the North Shore, Hibiscus Coast and Warkworth/Wellsford regions, supporting them at home, in one of our two inpatient units, or in community programmes offered at each of our three sites. Our specialist services, given free of charge, include medical care, spiritual, cultural, psychological and social support, physiotherapy, occupational and complementary therapy, social activities and more. We are a progressive organisation that puts each patient, whanau and family, our community and our people, at the heart of everything.
