An opportunity to join our team has become available for a 1 year fixed term.
Reporting to General Medicine Head of Department and strongly supported by a tight knit team, you'll be focusing directly on providing care to our elderly population. As well as addressing general geriatrics, there will be a focus on the provision of care to functional, frailty and hospice medicine.
This 0.6 FTE position will work with the rehab team to admit, round and discharge patients from the rehab service. It is also expected that this position will have admitting and discharge responsibilities for two inpatient hospice beds. They will work with Hospice Tairawhiti and their nursing service to provide consultation for in hospital referrals.
Additionally, there will be an expectation to provide baseline geriatric services, such as frailty and delirium assessments, medication reconciliation, functional and ambulatory assessments and recommendations, and participation in the discharge coordination of these patients.
Lastly, this position will be responsible for delirium and frailty assessments for patients admitted under our hip fracture service.

The remuneration range for this role is $170,369 - $250,560 as per ASMS Multi-Employer Collective Agreement for SMO's.
Minimum Requirements:
You will possess a general or vocational registration in general practice, general medicine or geriatric medicine.
Interest or training in geriatrics, hospice, or ambulatory medicine.
Working knowledge of inpatient rehab processes.
General knowledge of community services available for the elderly in the Tairawhiti region.
Relationship with Hospice Tairawhiti.
