This is a temporary position for up to one (1) year, with potential to renew subject to a successful pilot. The Complex Surgical Coordinator role aims to improve the coordination and management of patients pre-operatively in order to gain benefits in the pre, peri and post-operative phases of the patient's surgical journey. General Oversee management of complex patients through their surgical pathway, from the point of being waitlisted for surgery to being discharged from the hospital. Pre-Admission Ensure that patients with complex health or social needs have care plans in place, and achieve their "prehabilitation" goals in order to be as optimised as possible for their elective surgery journey. Wards Continue to actively manage patients with complex health or social needs through their ward stay, with a view to facilitating timely discharge and reducing average length of stay.
