Community Support Worker Physical Health and ClozapineTees Esk and Wear Valleys NHS Foundation TrustWe are looking to appoint a female support worker due to the needs of the service.
This is a Genuine Occupational Qualification (GOQ) in accordance with the Equality Act 2010. This is an excellent opportunity to be part of delivering care to new and existing patients that require Physical Health Monitoring within the South Dales Treatment and Intervention Team at the Goodall Centre, Bishop Auckland. You will not only be part of a Physical Health Team but part of the wider community teams based in the Goodall Centre.
The main role of this post is to complete general physical observations (BP, Pulse, etc.), ECGs, and venepuncture. Experience and skills in this aspect are essential to the role, but training can be provided for the right candidate. The Physical Health clinic and functioning are supported by the leadership team.
As part of the Physical Health Team, you will attend the daily huddles and MDT, liaise with prescribers and colleagues in the team to ensure safe practice and care.
Main Duties of the JobThe weekly tasks will involve the organisation and running of the Physical Health Clinic along with your colleagues. You will be required to book and follow up appointments, maintaining accurate patient records and spreadsheets to ensure robust monitoring of patient medication.
Where home visits are required, you will organise these by liaising with the patients, carers, and others.
The Physical Health Team takes responsibility for the clozapine clinic which runs every Tuesday. You will be provided with specialist training to operate and maintain the Pochi Machine. The Clinic works closely with the pharmacy staff to ensure that the patients on clozapine receive safe and appropriate care.
QualificationsDiploma in Health and Social Care level 2 or equivalentCare CertificateKey skills in Literacy and Numeracy level 2Successful completion of Trust's full HCA Framework within a 6-month probationary periodExperiencePrevious experience working with people with mental/social health needs or learning disabilitiesExperience of working in Physical Health, HDAT, Lithium, and Clozapine ClinicsExperience in venepunctureExperience of using a Clozapine Pochi machineSkillsAbility to take general observations (i.e., pulse, BP, temperature, weight, and height)Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and as such, it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
