Nursing - Community, Maternal & Child Health (Healthcare & Medical)
Presbyterian Support is one of the largest providers of social and health services in New Zealand. Our East Coast team is made up of over 200 staff and 100 volunteers across our services, Enliven and Family Works in Hawke's Bay, and two Charity Shops.
Enliven Intensive Home Support, a service of Presbyterian Support East Coast, provides support services for older people so they can live and enjoy life as a valued part of our community.
We currently have one permanent part-time position 0.8FTE (4 days per week) available for a Community Nurse Manager. This is a case management role supporting service delivery for older people with complex health needs and long term chronic conditions, to be supported at home in the Hawke's Bay community.
About the role: Our Community Nurse Manager works collaboratively with the guidance of the Enliven multidisciplinary team, with the primary goal of building functional abilities and maximizing independence of the client. You will be joining a team that strives for excellence and works with a heart for the people we serve, and each other.
Your role will include:
Exceptional ability to liaise, build and maintain professional relationships with clients, their family and whanau.
Provide safe clinical judgement, reasoning and outcomes in practice.
Providing case management of complex clients, clinical assessments, and identifying clients needs and package care for each client and their family and whanau.
Creating support plans to meet complex client needs while maximising independence, settling S.M.A.R.T achievable goals, ensuring regular, and as needed reviews in a timely way.
Ensuring clear, concise, accurate communication and documentation.
Efficient time management of care resources in largely administrative role.
Building and maintaining positive relationships internally and externally.
What you will bring -
As a Community Nurse Manager (Registered with the Nursing Council of New Zealand), you will bring your valuable clinical nursing knowledge and experience to our team and community.
You'll have:
Exceptional technical skills, manage technology and systems remotely, for case management workload.
Excellent documentation and communication skills.
Highly developed clinical skills and implementation of bicultural practice.
Be a team player, sound individual practitioner, and very reliable.
Builds and maintains positive functional relationships internally and externally.
Manage resources well within available capacity.
You will need:
Experience working with older people with complex health needs.
Be a qualified and Registered Nurse.
Full clean NZ drivers license.
Community experience preferred but not essential.
