In this position, you will provide excellent holistic case management in partnership with the clients, their families/whanau, and carers, with a strong focus on returning clients to the primary health care sector following specialist input.
In this role, you will be responsible for:
The Case Management portion of the role will consist of the oversight and management of a varying case portfolio.Completing assessments of consumers and formulating interventions to meet individual needs in collaboration with our multi-disciplinary team (MDT).Your skills will include:
Experience in mental health is essential; experience in case management is ideal.The ability to triage and prioritize your patients appropriately in a timely manner is essential.A minimum of 2 years' experience in a mental health setting is preferable.Registered with the Nursing Council of New Zealand with a current APC and must have a mental health scope of practice.Specialist Mental Health Services are an interdisciplinary team that forms part of Health NZ | Te Whatu Ora - Canterbury | Waitaha.
