Tees Esk and Wear Valleys NHS Foundation TrustWe are seeking a highly motivated Community Mental Health Nurse on a full time, permanent basis.Nursing applicants should already hold NMC registration. Please note: Newly qualified Nurses or those due to qualify will be recruited via a separate process.Promotes and maintains safety, privacy and dignity of all patients in the delivery of patient centred care, recognising and respecting differences including spiritual and cultural beliefs.Takes steps to obtain patient consent to care and treatment.Responsible for recognising the potential for or signs of patient harm, abuse or neglect.Responsible for ensuring the highest professional standards and attitudes towards the care of patients are maintained at all times.Has responsibility for own caseload of patients and takes responsibility as lead professional or care co-ordinator as appropriate.Assesses patients including those with complex needs and develops, implements and evaluates intervention plans.Ensures intervention plans are based on current risk assessment and evidence based practice.Undertakes risk assessments in accordance with the Trust's Clinical Risk Assessment and Management Policy, devising and implementing actions and intervention plans.May be required to accompany and provide support to patients within a variety of community settings, dependent on current risk assessment.Main duties of the jobThere is no age barrier to becoming unwell in later life with both mental and physical health problems. Our progressive Mental Health Services for Older People (MHSOP) deliver high quality, accessible, person-centred care to the population of older people which the Trust serves.The service offers assessment, diagnosis and treatment for older people who are suffering from an organic illness, alongside complex physical health problems or related frailty.We have a multi-disciplinary community team including nurses, medical staff, advanced nurse practitioners, physiotherapy, occupational therapy and psychology. During the discussion we can consider any development needs you may have and help you decide if the post would meet your long term career plans.Please refer to the attached job description and person specification for further detailed information to ensure that you meet the role criteria before applying.Person SpecificationQualificationsCurrent professional registration with Nursing & Midwifery Council RNMH or RNLD (RNSC - CAMHS only) - RGN (If appropriate).To be recognised practice assessor / mentor or work towards in an agreed timescale.Trained Clinical Supervisor (within 6 month probationary period).Evidence of Continuing Professional Development.ExperienceWorking with people with mental ill health / learning disabilities.Working in a person centred therapeutic organisation.Working collaboratively with service users and their families/carers.Working in a multi-disciplinary team.Working in partnership with other agencies.KnowledgeEvidence based knowledge of best practice in caring for patients in the designated field.Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).Understanding of Safeguarding and its application in practice.Clinical Risk Assessment and Management and its application in practice.Clinical Governance and its application in practice.Confident working knowledge of clinical medicines management, including administration of injections.Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
