Your responsibilities will include:
Collaborating with multidisciplinary teams to develop and implement personalized care plansDeliver, monitor, and review patient health and wellbeing needsAdvocating for patients and their families to ensure their needs are metProviding support and guidance to navigate the healthcare systemContinuously assessing and reassessing patient needs to optimize care deliveryLead the IDT functioning in collaboration with relevant health and social service providers and maintain relationships within the practice team, wider community and secondary care providers, external agencies, and NGOsWhat We Need: A Registered health professional with current APC and no restrictionsComprehensive understanding of inequities in access and health outcomes in Aotearoa, New Zealand and detailed knowledge of Te Tiriti o Waitangi and its application to shaping all efforts within primary careStrong cultural competency and confidence with sufficient knowledge of the peoples, languages and cultures that form our most at-risk communitiesWhy ProCare?
