The Clinical Pharmacist will work primarily in the Pre-admission Department with a focus on medicine reconciliation and medication optimization. The Clinical Pharmacist will also support medication management of complex patients in a multi-disciplinary team focused on "pre-habilitation" care plans and milestones prior to surgery. Patient education focused on pre-operative medicines management is another key aspect of this position.
You will have the opportunity to:
Optimise patients prior to planned surgery Manage pre and post-operative medication Work as part of an MDT to prepare patients for surgery Have patients and family/whanau/carers as the primary focus Be proactive and transparent in the management and delivery of pre-anaesthetic care Optimise continuity of care for patients Empower staff to participate in decision making Promote the most effective use of available resources Have an interdisciplinary approach Are you a qualified Clinical Pharmacist who can practise in New Zealand and who would embrace an exciting opportunity in a beautiful province with fantastic work-life balance?
Ideally, the candidate will have:
