The Clinical Pharmacist will work primarily in the Pre-admission Department with a focus on medicine reconciliation and medication optimization. The Clinical Pharmacist will also support medication management of complex patients in a multi-disciplinary team focused on "pre-habilitation" care plans and milestones prior to surgery. Patient education focused on pre-operative medicines management is another key aspect of this position.
