Based at North Shore Hospital

Each year, thousands of New Zealanders needlessly experience debilitating fractures because the underlying cause of their broken bones — osteoporosis — was undetected and untreated. FLS systematically identifies, treats, and refers to appropriate services all eligible patients aged over 50 years who have suffered a fragility fracture, with the aim of reducing their risk of subsequent fractures.
As an FLN, you will be required to:

Identify all patients over the age of 50 with fragility fractures.
Initiate contact with patients either face to face or by phone to arrange assessment, blood tests, DEXA scan, and develop a plan of care.
Develop a treatment plan for those older patients who do not require a DEXA scan and are at high risk of future fractures.
Report on patients seen, including review of results and dates for follow-ups.
Refer patients to other specialties.

Undertake training to be able to complete DEXA bone density scanning for Waitemata patients.
The successful candidate must demonstrate the following:

Be a level 4 registered nurse with at least 5 years post-registration experience, preferably in trauma and orthopaedics, care of the elderly, or rheumatology.
Knowledge of osteoporosis, fall prevention, multi-pathology, and polypharmacy would be advantageous.
Able to work autonomously but also as part of a team.

The successful person would need to be self-motivated, enthusiastic, with exceptional organisational and communication skills, as well as being flexible, adaptable, and assertive.
The preferred candidate may be placed on the specialty nurse development program to support their transition to the expectations of the role if not currently functioning at a CNS level.
