The Diabetes Clinical Nurse Specialist (CNS) provides specialist nursing advice, care and expertise, in thedelivery of patient care and supporting other members of the multidisciplinary team, across a variety of settings;this will include primary and secondary care. The focus is on assisting, directing and supporting the care of people with diabetes through role modelling expert clinical skills, promoting best practice, facilitating interdisciplinarycollaboration and providing education. The ideal candidate will be a senior nurse who is motivated to workindividually and as part of a team, who is consistently striving to identify strategies and quality initiatives aimed atimproving the long term health outcomes for people with diabetes. This will include a wide range of activities fromindividual patient care to the development and review of guidelines and policies; both at service specific level andwhen indicated, national guidelines. Demonstrated confidence and the ability to deliver quality education acrossthese settings is considered a core competency for this role. Consideration will also be given to the rightcandidate, not currently a Diabetes CNS, but who has the necessary skills and clinical experience to be supportedto become an accredited Diabetes Nurse Specialist and RN Prescriber over the next 4-5 years.
We are a team of eight Clinical Nurse Specialists, of which six are currently Designated Nurse Prescribers. We have a full time specialist Diabetes Dietitian and access to independent psychology services.
Four senior adult and three senior paediatric physicians support us clinically.
We are passionate about delivering an inclusive, equitable and responsive service for our community with a focus on both self-management and shared care. We work collaboratively and pool our skills and knowledge when required, to enable best outcomes for those we are working alongside. We promote the use of technology when appropriate and are skilled in pump therapy management and ongoing review.
Each of our Nurse Specialists is responsible for a particular portfolio and is affiliated with a number of GP practices in the area. We develop and maintain robust relationships both in the community and within the hospital setting, with liaising, supporting and educating our Primary Health Care and hospital teams regularly through the year.
Mou - About you
It is an expectation that, in the first few years the successful candidate will be exposed to all aspects of complex diabetes management including, inpatient care, gestational management, paediatric diabetes, young-adult management and diabetes technology such as insulin pump therapy. This model supports the way the Hutt Diabetes Service Team works on a day-to-day basis. Wherever possible, further down the track, we will endeavour to more specifically tailor the role to the individual candidate - s particular passions or interests.
