Location: Based in either Whangarei, Bay of Islands or Dargaville

Role Type: Permanent role at a minimum of 0.8 FTE up to full time 1.0 FTE

About the Role and Service

NASC provides Needs Assessment & Service Coordination for older people and people with chronic health conditions with high needs who have a disability requiring long term supports, and for people of any age who require acute short-term community home support.

These positions, minimum of 0.8 FTE (32 hours per week) are based in either Whangarei, Bay of Islands or Dargaville Hospital with travel throughout Northland.

Primary functions of the role include providing screening and triaging of referrals and providing the coordination of the appropriate services identified in the needs assessment.

About You

Essential Skills & Experience

Tertiary qualification in either Nursing, Occupational Therapy, Social Work or Physiotherapy with a current practising certificate

Recent clinical experience with proven assessment skills

Excellent written and verbal communication

Advanced computer literacy

Clean and current NZ full driver licence

Competency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growth

Experience in projects/initiatives which give effect to Te Tiriti principles and frameworks, and the application of Matauranga Maori and Kaupapa Maori approaches, particularly as they apply in healthcare settings.

Desirable Skills

Working knowledge of services for older people and the disability sector

Experience working with Family/whanau in the community

Confidence in previously working autonomously and harmoniously within a multidisciplinary team
