Through integration and collaboration, we're building a health system underpinned by partnership, equity, sustainability, whanau-centred care, and excellence.About the Role and ServiceNASC provides Needs Assessment & Service Coordination for older people and people with chronic health conditions with high needs who have a disability requiring long term supports, and for people of any age who require acute short-term community home support.These positions, minimum of 0.8 FTE (32 hours per week) are based in either Whangarei, Bay of Islands or Dargaville Hospital with travel throughout Northland.Primary functions of the role include providing screening and triaging of referrals and providing the coordination of the appropriate services identified in the needs assessment.About youTertiary qualification in either Nursing, Occupational Therapy, Social Work or Physiotherapy with a current practising certificateRecent clinical experience with proven assessment skillsAdvanced computer literacyClean and current NZ full driver licenceCompetency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growthExperience in projects / initiatives which give effect to Te Tiriti principles and frameworks, and the application of Matauranga Maori and Kaupapa Maori approaches, particularly as they apply in healthcare settings.Working knowledge of services for older people and the disability sectorWorking with Family/whanau in the communityConfidence of previously working autonomously and harmoniously within a multidisciplinary teamWorking for Health New ZealandHealth New Zealand | Te Whatu Ora is 'the weaving of wellness'.
