It's about coming to work every day, feeling challenged, trusted and supported, whilst enjoying living in a region that has a lifestyle unrivalled in New Zealand.The role:We have opportunities for experienced Health Professionals with a current practicing certificate to join our Whangarei Needs Assessment and Service Coordination (NASC) Team based in either Whangarei, Bay of Islands or Dargaville.NASC provides Needs Assessment & Service Coordination for older people and people with chronic health conditions with high needs who have a disability requiring long term supports, and for people of any age who require acute short-term community home support.These positions, minimum of 0.8 FTE (32 hours per week) are based in either Whangarei, Bay of Islands or Dargaville Hospital with travel throughout Northland.Primary functions of the position include:To provide screening and triage of referrals.To provide a needs assessment identifying requirements for specialist assessment, therapeutic intervention and ongoing support needs for people in the following groups:65 years of age and over, or with appropriate age-related disabilities,people between the ages of 16 and 64 years, with chronic health conditions.When required; to provide the co-ordination of the appropriate services that are identified in the need's assessment.Requirements:Tertiary qualification in Nursing, Occupational Therapy or Physiotherapy with a current practising certificateAt least 5 years post graduate experienceAdvanced computer literacyClean and current full driver licenseConfidence of previously working autonomously and harmoniously within a multidisciplinary teamWorking knowledge of services for older people and the disability sector is advantageous.Health New Zealand / Te Whatu Ora Te Tai Tokerau is committed to Te Tiriti o Waitangi and holds stewardship for equitable health outcomes for Maori as Tangata Whenua as the highest priority.Our people live our Values:Tangata I te tuatahi People First - People are central to all we doWhakaute (tuku mana) Respect - We treat others as they would like to be treatedManaaki Caring - We nurture those around us, and treat all with dignity and compassionWhakawhitiwhiti korero Communication - We communicate safely, openly and with respect to promote clear understandingTaumata teitei (hirangi) Excellence - Our attitude of excellence inspires success, competence, confidence and innovationVacancy No: AH24-068, AH24-069Closes: 28 August 2024Employer: Te Whatu Ora - Health New Zealand Te Tai Tokerau
