We are looking for a Clinical Needs Assessment and Service Coordinator (NASC – Clinical) to join their team.

The Kaupapa Maori Needs Assessment Service Co-ordination (NASC) is a free community service for adults aged between 18 to 65 years. It offers access to a wide range of community-based resilience, recovery, and support-focused services.
Reporting to the Service Delivery Manager or Appointee, this position is aimed at ensuring eligible whanau have timely access to high quality mental health and addiction support services and other support where identified. Assessments will meet the unique needs of individuals and whanau by being flexible around service delivery settings both in the home, in an appropriate community setting and office based and adaptable in meeting the cultural and spiritual needs of individuals.
Key responsibilities of the role:
Process and manage all internal and external referrals, meeting required timeframes
Complete Client Needs Assessments and prepare personalised support plans
Provide personalised packages of support for clients which are recovery focussed
Facilitate access to support services, arrange referrals to external agencies for specialist assessments and treatments
Liaise with external agencies and treatment providers
Complete reporting requirements, ensuring strict client confidentiality
File management, capture data and update progress details

Minimum of three years' experience in a similar role
Good understanding of Kaupapa Maori frameworks and application to service delivery
Knowledge of TKKH and the Eastern Bay of Plenty Communities
Good understanding of Te Ao Maori
Strong relationship and network management
Excellent written and verbal communication skills
Good interpersonal, team building and conflict resolution skills
Can effectively influence and lead others
Demonstrates a strong personal commitment to organisational excellence, honesty, integrity, and business ethics
Excellent organisational skills, with the ability to meet deadlines
Excellent computer literacy skills, with proficiency in all Microsoft Office functions
Proven experience working in a Kaupapa Maori Health Organisation is desirable
Registered practitioner/social worker with a current practicing certificate would be desirable but not essential
Knowledge and understanding of Te Reo is desirable but is not essential
