Established in 1995, our client Tuwharetoa ki Kawerau Hauora provides quality health, education, and social services within the rohe of Tuwharetoa i te Aupouri and the Eastern Bay of Plenty communities that reflects te taha tinana, te taha hinengaro, te taha wairua me te taha whanau.  We are looking for a Clinical Needs Assessment and Service Coordinator (NASC – Clinical) to join their team.

The Kaupapa Maori Needs Assessment Service Co-ordination (NASC) is a free community service for adults aged between 18 to 65 years. It offers access to a wide range of community-based resilience, recovery, and support-focused services.

Reporting to the Service Delivery Manager or Appointee, this position is aimed at ensuring eligible whanau have timely access to high quality mental health and addiction support services and other support where identified. Assessments will meet the unique needs of individuals and whanau by being flexible around service delivery settings both in the home, in an appropriate community setting and office based and adaptable in meeting the cultural and spiritual needs of individuals.

Key responsibilities of the role: Process and manage all internal and external referrals, meeting required timeframesComplete Client Needs Assessments and prepare personalised support plansProvide personalised packages of support for clients which are recovery focussedFacilitate assess to support services, arrange referrals to external agencies for specialist assessments and treatmentsLiaison with external agencies and treatment providersComplete reporting requirements, ensuring strict client confidentialityFile management, capture date and update progress details Nga pukenga me whai | Ideal candidate requirements Minimum of three years' experience, in a similar roleGood understanding of Kaupapa Maori frameworks and application to service deliveryKnowledge of TKKH and the Eastern Bay of Plenty CommunitiesGood understanding of Te Ao MaoriStrong relationship and network managementExcellent written and verbal communication skillsGood interpersonal, team building and conflict resolution skillsCan effectively influence and lead othersDemonstrates a strong personal commitment to organisational excellence, honesty, integrity, and business ethicsExcellent organisational skills, with the ability to meet deadlinesExcellent computer literacy skills, with proficiency in all Microsoft Office functionsProven experience working in a Kaupapa Maori Health Organisation is desirableRegistered practitioner/social worker with a current practicing certificate would be desirable but not essentialKnowledge and understanding of Te Reo is desirable but is not essential Why Tuwharetoa ki Kawerau Hauora?
