Whakamarumarutia | Active Protection The Role: CEP's role is to provide education and support to those who face complex and chronic health barriers. They promote healthy lifestyle changes through a targeted 'service delivery' that works with the client(s) to ensure improved health outcomes. This includes (but not limited to), clinical assessment, monitoring and evaluation, in-home strength and balance programmes and physical activity programmes. The overall goal is to improve the client's health, reduce risk and the patient ultimately self-manages their physical activity.
The successful applicant will work with, and provide prescription based physical activity and education to individuals, groups, primary health care teams and health professionals within the Manawatu region. The role will see you directly working within the community and reaching those who may face barriers coming to a practice. As such, this role may see you going to the client to reduce barriers. The overall goal is to improve the client's health, reduce risk and the patient ultimately self-manages their physical activity delivered in a culturally appropriate way.
We are very interested if you are motivated and passionate about improving health outcomes in the primary care sector and have the below:
A Clinical Exercise Physiologist qualification with a minimum of 1-2 years' experience Minimum of 2 years' experience delivering education and support to priority populations The ability to motivate lifestyle change in people with long term conditions Ability to clearly communicate and adjust communication style to ensure understanding Experience teaching & educating clients, families, whanau & health professionals in a group environment or individually. Experience in sports and/or exercise science Ability to work autonomously and as part of a team Able to be proactive in helping to reduce barriers to physical activity and wellness such as home programmes. Full and unrestricted Drivers Licence Understanding of how the Principles of the Treaty of Waitangi relate to this role. Understanding the need for an equity focus on our population/communities health It would be highly desirable if you are well connected within the primary health community and have professional sport and recreation networks within the Manawatu region.
