Clinical Care Coordinator - Mental Health and Addictions ServiceLocation: Hamilton with some possible travel across the district
Hours: 80 hours per fortnight.
Part time hours will be considered

About the Role We are seeking four registered health care professionals (Registered Nurse, Occupational Therapist or Social Worker) to join two new mental health community teams.
The primary purpose of the roles is:
Facilitating tangata hapori/ tangata whaiora transitions throughout the service system.Improving the flow of information amongst everyone identified in the tangata hapori/ tangata whaiora's circle of support.Monitoring the systems and processes that underpin the provision of good quality care and support.Key responsibilities include: Working with tangata hapori/ tangata whaiora, their whanau and providers to produce plans together, to best meet their needs and preferences.Coordinating the tangata hapori/ tangata whaiora's plan to direct, enable and evaluate a range of interventions, from a range of providers to meet goals identified.Developing and building strong interagency relationships across Iwi, Maori NGO providers, Government organisations, Non-Governmental Organisations and the Mental Health and Addictions Sector.About You Competency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growth.Experience in upholding Te Tiriti principles and frameworks, and the application of Matauranga Maori and Kaupapa Maori approaches, particularly as they apply in healthcare settings.Whanau centred, collaborative and partnership approach.Experience working in a mental health, community health or primary care setting.Experience working with Maori and Pasifika groups.Highly developed interpersonal communication and negotiation skills including demonstrated ability to build productive working relationships with colleagues to achieve required outcomes.Ability to take a proactive role with other organisations to achieve sustainable outcomes for tangata whaiora.Registration with the applicable registration board with a current annual practicing certificate.Working for Health New Zealand Health New Zealand | Te Whatu Ora is 'the weaving of wellness'.
