You will work with our patients and their families/whanau to determine the level of care and support required when looking at facilities upon discharge from hospital.You will be InterRAI trained to effectively assess the needs of our patients and have a strong interdisciplinary approach.The Clinical Assessor is part of the interdisciplinary team and is the key link in the care chain from the ward to Aged Residential Care.
