We have an exciting new opportunity for a Registered Nurse or Allied Health Professional to work across our Older Persons Health inpatient wards at Burwood Hospital.
Burwood Hospital is a small friendly facility situated near Burwood forest. We have excellent working conditions and parking is free!
Pehea | About You
Working as a Clinical Assessor is about assessing our patients' needs. You will work with our patients and their families/whanau to determine the level of care and support required when looking at facilities upon discharge from hospital.
You will be InterRAI trained to effectively assess the needs of our patients and have a strong interdisciplinary approach.
The Clinical Assessor is part of the interdisciplinary team and is the key link in the care chain from the ward to Aged Residential Care. In this role, you will understand how important it is to be patient-focused.
Nga pukenga me nga wheako | Skills and experience
Comprehensive clinical assessment and problem-solving skills to formulate the patients' care plan
Must have a professional degree/qualification, and/or be a Registered Health Practitioner (under the HPCA), or overseas equivalent (Social work, Occupational Therapy, Registered Nurse, or related discipline)
Prior completion of the InterRAI Assessment training is desirable (or be prepared to complete)
Have completed InterRAI Assessment training (or be prepared to complete)
Excellent communication and interpersonal skills
Effective time management and organisational skills
Clinical experience in the specialty area of gerontology
