Where you'll be working: Waikato Hospital The details: 6 months fixed term, 48 hours per fortnight Salary details: Grade 3 Senior Nurse Scale, NZNO Te whiwhinga mahi | The Opportunity We are seeking an experienced Senior Nurse to join our WHIRI (Whānau Hauora integrated response initiative) rōpū, here at the Māori Health directorate Te Whatu Ora Waikato.
As a successful Nurse Charger Nurse Manager (CNM) you will play a key role in managing the Hospital WHIRI clinical team of CNS, Kaiwhiriwhiri (Health Navigators), and working with GP, Clinical Equity leads and reporting to the Equity Programme Manager.
You will drive equity and this programme of work will embrace Mātauranga Māori and Te Ao Māori in order to be effective for our Māori communities and fulfil our Te Tiriti obligations.
Day to day you will job share with another Team Leader to manage the WHIRI team, have a small patient load, build relationships with OPC (Outpatient Clinic Services), Team leader and Booking Clerks.
You will manage the roll out of the Planned Care OPC support to services, coordinated primary and secondary healthcare, Social services/care and to identify and address unmet needs.
You will be Developing pathways and processes.
This is a 0.6FTE, Fixed Term 6 month position where you will work Wednesday to Friday 8 hours daily, with flexibility to work 1 day from home.
Key responsibilities of the role: * Manage nursing staff and health navigators, workflow and workloads across the clinical area by ensuring appropriate staff skill mix, nursing process and risk mitigation.
* Clinical leadership to establish and maintain a high standard of patient centre and whānau centred care and coordination.
* Monitor oversight of patients with high clinical risk and patient care coordination, clinical governance, patient safety.
* Role model expert clinical practices in the care and management of patients.
