Diverse role, interacting with a wide range of stakeholders Monday to Friday, between 8am and 5pm Opportunity to work from Porirua and Hutt Valley Supportive team Care Coordination currently has positions available within our Capital and Coast team (based in Porirua) and Hutt Valley team. As a Care Manager, no two days are the same. You will undertake assessments in the community using the interRAI suite of tools and your clinical expertise. Working in partnership with the client, their caregiver/family/whanau, you will be developing plans to enable older clients with complex needs to remain safely in the community. Education and communication with other providers are pivotal to this role. He momo/About you To be successful in this role you will have: Be a registered health professional (including assessment within your scope of practice). This could be a Registered Nurse or Allied Health Practitioner Have a high level of computer literacy Have experience in working with older people and/or chronic care management in a multi-disciplinary environment Be available to work 32 - 40 hrs per week (Monday - Friday) Experience in clinical/social needs assessment, older person's health, rehabilitation, or primary care is desirable. A current NZ practising certificate (or equivalent) and driver's licence are essential. A comprehensive orientation programme is in place to ensure you receive training on the use of interRAI and other areas that are new to you.
