Based in beautiful Whanganui, Te Oranganui is a kaupapa Maori organisation that offers a range of health and wellbeing services for whanau in the Whanganui region and surrounding districts.
Te Waipuna Health is the General Practice arm with an enrolled population of 5,300 spread across two Practices. As a low-cost access health centre, we offer a newly refurbished Practice in a supportive and flexible environment.
About the role:The role of Care Coordinator focuses primarily on supporting people and their whanau who are at the greatest risk of poor health outcomes, particularly Maori and Pasifika who require their care to be transferred between providers and across services including secondary care. This includes navigation and advocacy with relevant hauora services and education, housing, justice, cultural connection, spiritual, and environment services where relevant.
About you:You will have the ability to connect compassionately and collaboratively with whanau and health care professionals. You will have a commitment to improving health outcomes for whanau. Being flexible, adaptable, and a critical thinker will be additional strengths as you'll find that no two days are the same. You will have the ability to connect with different services across the rohe.
