Ko te aroka o te turaka - Position Purpose:
The Calderdale Coordinator will oversee the implementation and support of the Calderdale Framework within WellSouth. This pivotal role will focus on facilitating the development and refinement of Clinical Task Instructions (CTIs) with clinicians, ensuring the efficient and effective integration of non-registered workforces, such as Health Coaches, Community Support Workers, Coordinators, Kaiawhina and Health Care Assistants.
By streamlining the Calderdale implementation, the coordinator will enable other team members to focus on their core responsibilities, enhancing the capacity and capability of primary health care services.
Mo te turanga - About the role:
Coordinate and manage the implementation of the Calderdale Framework across various WellSouth teams and GP practices, collaborating with Te Whatu Ora services where necessary.
Organise and oversee Awareness raising sessions and Foundation Day.
Support the ongoing development and training of CTIs, prioritising areas that can improve the efficiency and effectiveness of patient care.
Facilitate collaborative reporting of Calderdale Framework.
Develop and provide additional resources and education to support the Calderdale process.
Collaborate with clinicians to create, review, and develop CTIs that meet Calderdale Framework guidelines and address primary healthcare team needs.
Work with practices to identify training needs and opportunities, enabling the non-registered workforce to contribute more effectively to patient care.
Mo koe - About you:
We are looking for someone who:
Holds a relevant health/social services bachelor's degree (preferred); and/or
Possess a Level 4 qualification in Health Wellbeing (or approved equivalent); and/or
10 years of experience in health and social services field.
Two years' experience working in Primary Care (desirable).
Change management and/or project management qualifications are an advantage.
Trained Calderdale Facilitator (desirable). Training will be provided.
Ability to coordinate and manage the implementation of frameworks across multiple teams and stakeholders and effectively engage with healthcare professionals to ensure alignment with clinical standards.
Strong time management skills and ability to work calmly during times of pressure.
